Cable TV ratings: How are they calculated?

For cable ratings, are viewing habits taken automatically by agreement with the cable and IPTV providers like Charter, Time Warner and Comcast? Or is it still done by providing a small group of people with a physical box that records viewing habits, which is then thrown into a formula that multiplies it???
